What this rule requires your agency to do
- 1An HHA must apply for a change of ownership when the form of legal organization changes, the HHA's title transfers to another party, a partnership dissolves, or a corporation is dissolved/merged/consolidated, or a change of ownership/lessee or establishment of a branch occurs. - 3An HHA must apply to the Department for a change of ownership at least ninety (90) days prior to the proposed date of change, using the initial licensing application form. (Subsection 03)(IDAPA 16.03.07.015 — Change of Ownership)
