What this rule requires your agency to do
- 1The manager or designee must conduct a client satisfaction review every 76 to 104 days, documented in writing and signed/dated.(Personal Services Agency License Requirements (IDOH-HCBC/PSA-1; IC 16-27-4))
- 2To operate a personal services agency a person must obtain a license from the state health commissioner (each parent PSA obtains a separate license; application requires a state form and a $250 fee); a PSA license may not be transferred or assigned, and upon sale/CHOW the new owner must obtain a license before operating.(Personal Services Agency License Requirements (IDOH-HCBC/PSA-1; IC 16-27-4))
