What this rule requires your agency to do

Requires Medicaid providers, including HCBS waiver and home health providers, to maintain complete, legible fiscal and clinical service records with specified content, sets service-specific documentation lists (including the HCBS Attendant Care Agreement and Attendant Care Service Record), governs corrections, and imposes a five-year retention minimum.
