Under 77 Ill. Adm. Code 245.210, an Illinois-licensed Home Services Agency must maintain an individual client record for each client, containing client and emergency-contact identifying information, the agreed plan of services, the home care services agreement, and documentation by the home services worker of each service provided at each visit. Records must be retained at least two years beyond the last date of service, may be kept in hard-copy or electronic form, and are governed by a written confidentiality policy.
