IDAPA 16.03.07.013 — Licensure - General Requirements

What this rule requires your agency to do

  • 1An application for a license must be made to the Department on its forms and contain the information it reasonably requires, including affirmative evidence of ability to comply with lawfully adopted standards and rules. (Subsection 03)(IDAPA 16.03.07.013 — Licensure - General Requirements)

Establishes Idaho's core home health agency licensure framework, requiring HHAs to meet the federal Conditions of Participation at 42 CFR Part 484 and setting out application, issuance, renewal, denial, revocation, and suspension standards. Licenses are non-transferable, must be posted conspicuously, and the Department may deny/revoke for endangerment, repeat deficiencies, insufficient qualified personnel, fraud, abuse/neglect, refusal of inspection, or loss of federal certification.
