Wisconsinsub_regulatoryWisconsin Department of Health Services (DHS), Division of Medicaid Services

What this rule requires your agency to do

  • 1EVV must collect, for each Medicaid-funded service code requiring EVV: who receives the service, who provides it, what service is provided, where, the date, and the time the service begins and ends.(Electronic Visit Verification (EVV): Providers and Independent Nurses)
  • 2Providers must train workers on how to check in and out of a visit using the EVV system, and (for Sandata) have the lead administrator add at least one additional security administrator in the Sandata EVV portal.(Electronic Visit Verification (EVV): Providers and Independent Nurses)
  • 3Providers using the DHS-provided Sandata EVV system must complete the entire self-paced administrator video training series to receive credentials and access to the Sandata EVV portal, and refer to the Wisconsin Supplemental Guide (P-02745) for Wisconsin-specific features.(Electronic Visit Verification (EVV): Providers and Independent Nurses)
  • 4Providers using an alternate EVV system must complete the steps on the Alternate EVV webpage and associate workers in the ForwardHealth Portal.(Electronic Visit Verification (EVV): Providers and Independent Nurses)
  • 5Providers must associate workers (or themselves, if an independent nurse) to their agency in the ForwardHealth Portal so workers can receive Sandata credentials and use the SMC app.(Electronic Visit Verification (EVV): Providers and Independent Nurses)
  • 6Providers (including independent nurses) new to EVV must update their provider contact information on the Demographic Maintenance area's Mailing Address panel in the ForwardHealth Portal and review the New to EVV? guide (P-03078).(Electronic Visit Verification (EVV): Providers and Independent Nurses)
  • 7Traditional methods of logging information, such as paper records of care, may continue but do not fulfill EVV requirements.(Electronic Visit Verification (EVV): Providers and Independent Nurses)

Applies to: personal care

Wisconsin DHS's EVV guidance for provider agencies and independent nurses, setting out the setup steps (ForwardHealth Portal contact update, worker association, Sandata administrator training) required to use an EVV system and stating that paper records of care do not fulfill EVV requirements.
