What this rule requires your agency to do

  • 1A client's medical need for Home Help must be certified on Form MDHHS-6200 or DHS-54A by a CHAMPS-enrolled Medicaid provider holding a listed professional license (now including clinical nurse specialist), or on VA Form 10-10M completed by a VHA provider.(MMP 26-09 - Updates to Home Help Program Policy)
  • 2The Home Help agency owner(s) must ensure all agency caregivers providing Home Help services successfully complete CHAMPS revalidation and are actively enrolled in CHAMPS at the time they provide services, and must monitor caregivers' revalidation status.(MMP 26-09 - Updates to Home Help Program Policy)
