- 4The provider must, when abuse or neglect is suspected, notify the department with a written report within 5 days describing the actions taken by personnel, the final outcome, and the actions taken to prevent a reoccurrence. - 5The provider must submit an initial written incident report to the department (by fax to 271-5574, or by regular mail postmarked within 2 business days if no fax is available) including the facility name, description of the incident and injuries, names of personnel/patient/witnesses involved, date and time, response and medical interventions taken, guardian notification status, and time the licensed practitioner was notified.
