What this rule requires your agency to do

  • 1Personal Care Services must be medically necessary and the need documented in the student's POC; each student's clinical record must contain a completed, signed and dated monthly activity log, including documentation of absences and days present when services were not provided.(MMP 25-56 - Updates to the MDHHS Medicaid Provider Manual (January 2026 Updates))
