What this rule requires your agency to do
- 1The provider agency must make a phone report by the next business day for a missing person, an untimely/suspicious death, and all other reportable incident types (802-241-2678). - 2The provider agency must report all defined critical incident types, including death, abuse/neglect/exploitation or prohibited practices, criminal act/incarceration, medical emergency, restraint (mechanical, physical, chemical), and suicide attempt. - 3The provider agency must notify all guardians, public or private, directly and immediately of any critical incident report, except where the guardian is the subject of an APS/DCF report. - 4The provider agency must submit reports through the designated channels: to DDSD via GlobalSCAPE FTP or fax (802-241-0410), and to ASD via SAMS or fax (802-241-0385). - 6The provider agency must make an immediate phone report for incidents involving potential media involvement occurring after hours, nights, or weekends. Applies to: Home and community based services (HCBS) provider agencies, supervisory agencies, and case managers serving individuals in DAIL waiver/HCBS programs
Vermont DAIL's Critical Incident Reporting Guidelines require HCBS provider agencies to report defined critical incidents (death, abuse/neglect/exploitation, missing person, medical emergency, restraint, suicide attempt, criminal acts, potential media involvement, etc.) by phone and in writing within set timeframes and to notify guardians immediately.
