What this rule requires your agency to do
- 1The individual must sign an agreement confirming they were informed of service alternatives, provider choice, and institutional vs. community options; if unable to sign at enrollment, must sign no later than the next in-person visit.(Rule 5160-46-02 | Ohio home care waiver program: eligibility and enrollment)
- 2The individual must participate in an in-person initial comprehensive assessment and cooperate with an in-person eligibility comprehensive assessment at least annually (more often on significant change).(Rule 5160-46-02 | Ohio home care waiver program: eligibility and enrollment)
