What this rule requires your agency to do
- 1A provider must implement services in the type, scope, amount, duration, and frequency specified in the service plan and document participant progress in the Department's designated information system.(55 Pa. Code § 52.14 - Ongoing responsibilities of providers)
- 2A provider must update and submit its provider qualifications under § 52.11(a)(3)-(7) to the Department at least every 2 years, and maintain and submit valid licenses/certifications each licensure period.(55 Pa. Code § 52.14 - Ongoing responsibilities of providers)
- 3A provider must ensure the service plan is Department-approved and the participant is eligible before rendering services, and may not render or bill for a service when the participant is unavailable.(55 Pa. Code § 52.14 - Ongoing responsibilities of providers)
- 4Within 180 days from the date of enrollment, a provider must attend new provider training provided by the Department.(55 Pa. Code § 52.14 - Ongoing responsibilities of providers)
- 5A provider must notify the Department at least 30 business days prior to changes in address/contact/name, articles/partnership agreements, audit revisions, license revocation/provisional status, or insurance cancellation (or within 2 business days if prevented by emergency).(55 Pa.
