What this rule requires your agency to do
- 1The provider agreement must be initiated by application from the individual agency or facility, expires on a specified date or with termination of the agency license or certification, and is automatically terminated on documented notice to OHCA that the agency or facility is not in compliance with Title XIX (or other federal long-term care) requirements. - 3Case manager certifications require successful completion of ADvantage case management training and demonstration of competency in case management and, for supervisors, case management supervision. - 4The Oklahoma Health Care Authority may execute agreements to provide care only with qualified individuals, agencies, and facilities which are properly licensed or certified by the state licensing or certification agency and, as applicable, Title XIX certified.
