What this rule requires your agency to do
- 1Develop and implement policies for handling drugs and biologicals (procurement, storage, administration, self-administration, disposal) and allow clients to procure medications from a pharmacy of their choice; dispense prescription drugs per Title 54.1 Chapters 33 and 34 and Board of Pharmacy regulations (subsections G, H).(12VAC5-381-300 — Skilled services)
- 2Document verbal orders within 24 consecutive hours in the client's record by the health care professional receiving the order, countersigned by the prescribing person (subsection C).(12VAC5-381-300 — Skilled services)
- 3Have the medical plan of care reviewed, approved, and signed by the primary care physician at least every 60 days (subsection E).(12VAC5-381-300 — Skilled services)
- 4Notify the primary care physician immediately of any changes in the client's condition indicating a need to alter the medical plan of care (subsection D).(12VAC5-381-300 — Skilled services)
- 5Have a director of skilled services (a Virginia-licensed physician or a registered nurse) responsible for overall direction and management of skilled services, availability, quality, and appropriate staffing, with appropriate experience for the scope of services (subsection F).(12VAC5-381-300 — Skilled services)
- 6Ensure all skilled services are prescribed in a medical plan of care containing diagnosis/prognosis, functional limitations, orders for all skilled services (procedures, modalities, frequency, duration), medication orders where applicable, and special dietary/nutritional orders where applicable; the plan must be approved and signed by the client's primary care physician (subsection B).(12VAC5-381-300 — Skilled services)
- 7Provide a program of home health services including one or more of the enumerated skilled services (nursing, physical/occupational/speech/respiratory therapy, or medical social services) (subsection A).(12VAC5-381-300 — Skilled services)
