What this rule requires your agency to do
- 1The caregiver may provide SFC to no more than three individuals residing at the same address, billed at the group rate when serving more than one.(Rule 5160-44-33 | NF-based level of care HCBS programs: structured family caregiving)
- 2The SFC provider must be a qualifying agency provider (a Medicare-certified home health agency or otherwise-accredited agency for OHCW, or an ODA-certified provider for PASSPORT).(Rule 5160-44-33 | NF-based level of care HCBS programs: structured family caregiving)
- 3Coaching staff must conduct monthly contact with the individual and caregiver, with no more than sixty calendar days between in-person visits.(Rule 5160-44-33 | NF-based level of care HCBS programs: structured family caregiving)
- 4SFC may not be authorized for individuals who are medically unstable or complex as a substitute for skilled care, nor provided on the same day as out-of-home respite.(Rule 5160-44-33 | NF-based level of care HCBS programs: structured family caregiving)
- 5The provider must employ coaching and support professional staff consisting of an RN, an LPN at the direction of an RN, an LSW, or an LISW.(Rule 5160-44-33 | NF-based level of care HCBS programs: structured family caregiving)
- 6For otherwise-accredited and ODA-certified agencies, the caregiver must successfully complete at least eight hours of initial training tailored to the individual's needs; the provider must maintain documentation that training was completed.(Rule 5160-44-33 | NF-based level of care HCBS programs: structured family caregiving)
