The HOPE Waiver provider manual governs assessment and reassessment for South Dakota's aged/disabled home- and community-based personal care waiver. A DHS LTSS Case Management Specialist completes the standardized Home Care Assessment (HCA) with the participant in the home, and an Individual Support Plan (ISP) must be developed collaboratively and completed at least annually or when needs change.
