COMAR 10.09.84.15 - Community First Choice: Covered Services - Supports Planning (Plan of Service)

What this rule requires your agency to do

  • 1The supports planning provider must assist the participant in developing a plan of service in consultation with the applicant or participant and any individual the participant requests to include.   • 2The supports planning provider must assist the participant with referral, access, and coordination of both Medicaid and non-Medicaid services needed to address the participant's needs, including behavioral health, educational services, disposable medical supplies and durable medical equipment, housing, medical services, and social services.   • 4The supports planning provider must ensure supports planning services address the individualized needs of the participant and are sensitive to the participant's educational background, culture, and general environment. Under Maryland's Community First Choice program, supports planning providers must develop each participant's plan of service in consultation with the participant and their chosen supporters, ensure the plan reflects individualized needs and self-direction, coordinate the identified Medicaid and non-Medicaid services, and monitor delivery to confirm services are furnished in accordance with the plan of service.
