What this rule requires your agency to do
- 1Agency caregivers and agency employees with direct client contact must carry and present a state- or agency-issued photo ID whenever they enter a client's home and whenever requested by MDHHS staff.(Adult Services Manual (ASM) 136 - Home Help Agency Providers)

- 3Agencies must revalidate their CHAMPS registration at least once every five years (or more often if requested by MDHHS), and must submit the CHAMPS application within 60 days of the application start date or be denied.(Adult Services Manual (ASM) 136 - Home Help Agency Providers)

- 5An agency selected for MDHHS's annual employment-document audit must submit the requested documents within 30 calendar days; failure within 60 calendar days results in removal from the approved agency list for at least 30 days or until compliance.(Adult Services Manual (ASM) 136 - Home Help Agency Providers)
- 6All agency owners, agency caregivers, and agency employees with access to clients' homes or personal information must undergo criminal history screening and be enrolled in and associated to the agency in CHAMPS (dated no earlier than completion of the criminal check) before providing Home Help services; the MSA-119 personal-choice option does not apply to agency caregivers.(Adult Services Manual (ASM) 136 - Home Help Agency Providers)
- 7Agencies must report all changes affecting enrollment (ownership, address, contact, caregiver/employee) by updating CHAMPS within 10 calendar days, or face termination, rate reduction, or claim denial.(Adult Services Manual (ASM) 136 - Home Help Agency Providers)
- 8Agencies must maintain supporting documentation verifying billed services (days/times worked, tasks completed, client names), keep the MDHHS-6064-P and any BPHASA-2421 Live-In Attestation, and retain records for seven years from the date of service, making them available to state/federal agents on request.(Adult Services Manual (ASM) 136 - Home Help Agency Providers)
