- 1The applicant must create and follow policies covering compliance, nondiscrimination, ADA, HIPAA, staff training, complaint management, critical incident management, quality management, criminal history screening, SSN verification, exclusion screening (LEIE/EPLS/Medicheck), and language access.(55 Pa. - 2The applicant must obtain and maintain appropriate licenses/certifications and obtain commercial general liability, workers' compensation, and (if required by profession) professional liability insurance.(55 Pa. Code § 52.11 - Prerequisites for participation)
- 3The applicant must verify fiscal solvency by submitting its most recent tax return, monthly balance sheet (or business plan), articles of incorporation/partnership agreement as applicable, and most recent audit/financial review if done within 5 years.(55 Pa.
