What this rule requires your agency to do

  • 1An invoice submitted to the FMS must contain the individual's name and medicaid ID, item/service description, vendor name, purchaser (if not the individual), purchase/delivery/service date, and paid amount.(Rule 5160-45-03.5 | ODM-administered waiver program: self-directed goods and services)
