What this rule requires your agency to do

  • 1The provider agency must limit the service plan to assistance with ADLs/IADLs directly related to the member's personal care needs to prevent or minimize physical health regression or deterioration (personal care, meal preparation, housekeeping, laundry, shopping/errands, or specified special tasks).   • 2The provider agency nurse or staff must contact the member within five (5) business days of receiving the authorized document to confirm service implementation. (OAC 317:35-15-8)(Okla. Admin.   • 3The provider agency must base each personal care service plan on a skilled nursing assessment of need and develop a care plan to meet the member's identified personal care needs.   • 4The provider agency must conduct monitoring visits at least every six (6) months using the Adv/SPPC Nurse Evaluation to assess member satisfaction and adequacy of the care plan, with a registered nurse conducting visits when Personal Care Assistants perform hands-on care.   • 5The provider agency nurse must reassess needs and develop a new service plan no sooner than sixty (60) calendar days before and no later than fourteen (14) calendar days prior to the service plan end date.   • 6The provider agency nurse must develop the service plan (including the Adv/SPPC Nurse Evaluation, SPPC Service Planning, and SPPC Member Service Agreement) within ten (10) business days of referral receipt and submit it to the Oklahoma Human Services Health Care Management Nurse.
