What this rule requires your agency to do

  • 1The provider must develop the list of required tasks to be performed by the aide based on the needs outlined in the SCDHHS Service Plan/Authorization, obtaining an updated Service Plan from the case manager prior to the start of services. The SCDHHS HCBS Personal Care Services scope requires provider agency nurses to conduct an initial in-home visit before services begin, build the aide task list from the case manager's Service Plan/Authorization, and perform supervisory visits within 30 days of initiation and at least every four months thereafter. Formal reassessment of the participant's underlying needs is performed through the case manager-developed Service Plan rather than independently by the personal care provider.
