What this rule requires your agency to do
- 1The provider must report as Tier 1 events all sexual abuse allegations, exploitation exceeding $1,000, unexpected or unexplained deaths, and alleged wrongful conduct by a service provider that resulted in harm requiring medical treatment or intervention. - 3The provider must provide verbal notification of a Tier 1 reportable event to the DDA Abuse Hotline (888-633-1313) within 4 hours of the event. - 4The provider must submit the written Reportable Event Form for a Tier 2 event the next business day to both DDA and the respective Managed Care Organization (MCO). - 5The provider must complete an investigation of a reportable event within 25 calendar days (or within 5 business days for ICF/IID facilities due to federal requirements). Applies to: home and community based services (HCBS) providers serving people supported through Tennessee's Medicaid waiver and CHOICES programs
Tennessee's Department of Disability and Aging Reportable Event Management (REM) framework requires HCBS providers to report critical incidents (reportable events) affecting people supported. Events are classified as Tier 1 (serious: sexual abuse, exploitation over $1,000, unexpected/unexplained death, wrongful conduct causing harm requiring medical treatment) or Tier 2 (less serious). Tier 1 requires verbal notice to the DDA Abuse Hotline within 4 hours plus a written Reportable Event Form (REF) by the next business day; Tier 2 requires a written REF the next business day to both DDA and the MCO. Providers must complete investigations within 25 calendar days (5 business days for ICF/IID). Applies to 1915(c) waivers, CHOICES, ECF CHOICES, and Katie Beckett.
