  • 1The agency must ensure the treatment orders are reviewed, as frequently as the patient's illness requires, by the physician, physician assistant, or advanced practice registered nurse in consultation with the agency.   • 2The agency must ensure treatment orders/the plan of care for a patient receiving skilled care are established by the attending physician, physician assistant, or advanced practice registered nurse acting within his or her scope of practice.   • 3The agency must ensure services are covered by an agreement between the agency and the patient or the patient's legal representative specifying the home health services to be provided, the rates or charges for services paid with private funds, and the sources of payment.
