  • 1The agency must ensure the treatment orders are reviewed, as frequently as the patient's illness requires, by the physician, physician assistant, or advanced practice registered nurse in consultation with the agency.   • 3The agency must ensure services are covered by an agreement between the agency and the patient or the patient's legal representative specifying the home health services to be provided, the rates or charges for services paid with private funds, and the sources of payment.
