What this rule requires your agency to do

  • 1Employee personnel records shall include the enumerated items (identifying info, education/training, employment history, license verification, references, evaluations, disciplinary and adverse actions, staff development, criminal record check and sworn affidavit) and be retained in entirety for a minimum of three years after termination (12VAC5-381-200.O, .Q).(12VAC5-381-200.   • 2All positive drug-testing results shall be reported to the applicable health regulatory boards pursuant to § 32.1-162.9:1 (12VAC5-381-200.P).(12VAC5-381-200. Personnel practices.)
  • 3The organization shall design and implement a staffing plan that reflects the services offered and provide qualified staff in sufficient numbers to meet the assessed needs of all clients (12VAC5-381-200.B).(12VAC5-381-200. Personnel practices.)

  • 7New employees and contract individuals shall be oriented to organization objectives, confidentiality, client rights, mandated reporting of abuse/neglect/exploitation, personnel policies, emergency preparedness, infection control, cultural awareness, and position-specific laws/policies (12VAC5-381-200.I).(12VAC5-381-200.   • 8All individuals who enter a client's home for the organization shall be readily identifiable by nametag, uniform, or other visible means (12VAC5-381-200.M).(12VAC5-381-200. Personnel practices.)
