What this rule requires your agency to do
- 1Direct providers of community-based services must keep all enrollee records confidential in controlled-access files for ten years from end of contract, and community-based providers must maintain records containing at minimum the request for services, pertinent enrollee information, a description of the service with units and cost, dates of service, and total cost.(MI Coordinated Health (MICH) Home and Community Based Services (HCBS) Waiver Requirements Resource Document)
- 2All nurses providing Preventive Nursing or Private Duty Nursing to enrollees must maintain a current State of Michigan nursing license and practice the standards under MCL 333.17201-17242; an LPN is responsible for securing an RN to supervise their work, and PDN requires a physician order that the HIDE SNP must ensure the provider has and delivers according to.(MI Coordinated Health (MICH) Home and Community Based Services (HCBS) Waiver Requirements Resource Document)
- 3HIDE SNP staff and direct providers of home-based services must receive in-service training as often as needed to ensure person-centered practices, must use MDHHS-developed training sessions and materials before their own, and must maintain comprehensive records identifying dates of training and topics covered in an agency training log and/or each employee's personnel file; the employer must develop an individualized in-service training plan for each employee when performance evaluations indicate a need.(MI Coordinated Health (MICH) Home and Community Based Services (HCBS) Waiver Requirements Resource Document)

- 7Service providers must have procedures for obtaining enrollee signatures on direct care workers' time sheets to verify the work ordered by the HIDE SNP was provided; direct service workers are prohibited from smoking in enrollees' homes, must be able to communicate adequately in writing and orally, and must not threaten or coerce enrollees (failure is grounds for immediate termination).(MI Coordinated Health (MICH) Home and Community Based Services (HCBS) Waiver Requirements Resource Document)
