What this rule requires your agency to do
- 1Each applicant and independent provider must pay BCI the prescribed fee for each criminal records check conducted on their behalf.(Rule 5160-45-08 | ODM-administered waiver programs: criminal records checks involving independent providers)
- 2Each applicant must, at the time of initial application for a Medicaid provider agreement, provide a set of fingerprints and undergo a BCI criminal records check as a condition of approval.(Rule 5160-45-08 | ODM-administered waiver programs: criminal records checks involving independent providers)

- 5Applicants/providers who were not Ohio residents for the prior five years (or lack an FBI check in that period) must have an FBI criminal records check requested as part of the check.(Rule 5160-45-08 | ODM-administered waiver programs: criminal records checks involving independent providers)

- 7To maintain active provider status, each independent provider must have their fingerprints submitted to BCI to be maintained in RAPBACK.(Rule 5160-45-08 | ODM-administered waiver programs: criminal records checks involving independent providers)
