OAC 310:662-2-1 - Application for and issuance of a home care agency license

What this rule requires your agency to do

  • 3The agency must submit a license application on forms prescribed by the State Commissioner of Health, including the full name and address of the applicant and all affiliated persons and the names, locations, and dates of ownership/operation/management for all current and prior home care agencies, accompanied by an attesting affidavit.   • 5The agency must demonstrate financial solvency with resources sufficient to ensure its ability to provide adequate home care services, and provide evidence of projected visits/shifts for six months and sufficient staffing availability.
