Massachusetts MassHealth regulations at 130 CMR 422.422 require Personal Care Management (PCM) agencies to conduct RN-led evaluations and reevaluations of members receiving personal care attendant services. Initial evaluations must be performed by an evaluation team including a registered nurse (or an LPN under RN supervision) together with an occupational therapist, conducted in the member's presence in the member's community residence and documented on the MassHealth evaluation form. Reevaluations must be conducted at least annually, or more frequently upon a significant change in the member's physical condition or living situation, by an RN or by an LPN or OT under RN supervision.
