  • 1The agency must include in its plan strategies for addressing identified emergency events, the patient population served, continuity of operations including delegations of authority and succession plans, and a process for cooperation with local, tribal, regional, State, and Federal emergency preparedness officials.   • 2The agency must develop and implement emergency preparedness policies and procedures, reviewed and updated at least every 2 years, that include plans for patients during a disaster, procedures to inform State and local officials of patients in need of evacuation, follow-up with on-duty staff and patients, and a system of medical documentation that preserves patient information and protects confidentiality.   • 3The agency must develop and maintain an emergency preparedness plan based on a documented facility-based and community-based all-hazards risk assessment, and review and update it at least every 2 years.   • 4The agency must develop and maintain an emergency preparedness communication plan containing contact information for staff, patients' physicians, and emergency officials, and primary and alternate means for communicating with agency staff and officials.   • 5The agency must provide initial emergency preparedness training to all staff and refresher training at least every 2 years, and must conduct exercises to test the emergency plan at least annually.
