HAR §11-700-4 — License

What this rule requires your agency to do
- 1Promptly surrender the license to the department when revoked or suspended, or when the agency terminates services (§11-700-4(g)).(HAR §11-700-4 — License)
- 2Permit authorized department staff to make unannounced inspection visits at any time (§11-700-4(h)).(HAR §11-700-4 — License)
- 3Notify each client concerned, or the client's responsible party, or both, at least thirty days prior to license expiration, voluntary surrender, or a revocation/suspension order (§11-700-4(g)).(HAR §11-700-4 — License)
