- 4A provider electing to use its own EVV solution must complete the NH EVV Third-Party Attestation Form and coordinate data submission of visit records with AuthentiCare; a provider using the state system must complete the NH EVV System Declaration Form. New Hampshire DHHS implements the federally mandated 21st Century Cures Act EVV requirement (SSA 1903(l)) for Medicaid personal care services. Providers must electronically verify each home care visit using an approved EVV solution — either the state-provided AuthentiCare system or a certified third-party system whose data is submitted to AuthentiCare. The EVV system must capture six data elements for every visit.
