Applies to: Home and community-based services / personal care (personal care visits, attendant care, community living supports) furnished under a person-centered plan of care
Under the TennCare CHOICES program rules, HCBS/personal care services must be furnished pursuant to an approved, person-centered plan of care (POC). The POC must identify the specific tasks and functions for which the member needs assistance, the frequency of those tasks, the member's need for safety monitoring/supervision, and the amount of paid assistance needed. CHOICES HCBS must be specified in the approved POC and authorized by the MCO before delivery.
