- 2Failure to submit all required information in the appropriate format subjects the agency to enforcement action under Section 12 of the Public Health Law, resulting in liability for civil penalties for violations of Article 36 (CHHAs and LTHHCPs) or Article 40 (Hospices).(2023 CHHA/LTHHCP Statistical Report and 2023 Hospice Cost and Utilization Report (DAL DHCBS 25-01))
