What this rule requires your agency to do
- 1Individual caregivers must submit Electronic Visit Verification (EVV) for Home Help visits per ASM 143.(Adult Services Manual (ASM) 135 - Home Help Caregivers)
- 2Every Home Help caregiver must enroll in CHAMPS and be approved, and must complete and pass a criminal history screen conducted by the MDHHS Provider Enrollment unit, before payment for Home Help services can be authorized.(Adult Services Manual (ASM) 135 - Home Help Caregivers)
- 3A caregiver must be at least 18 years of age; responsible relatives, minors (17 and under), and fiscal intermediaries may not be paid as the caregiver.(Adult Services Manual (ASM) 135 - Home Help Caregivers)
- 4The client and/or caregiver must notify the adult services worker within 10 business days of any change, including hospitalizations, nursing home or adult foster care admissions, and change or discontinuation of the caregiver.(Adult Services Manual (ASM) 135 - Home Help Caregivers)
- 5The client and the individual caregiver/agency provider must sign the MSA-4676 Home Help Services Agreement before payment is authorized, and caregivers must complete/agree to the MSA-4678 provider agreement terms (electronically in CHAMPS or on paper).(Adult Services Manual (ASM) 135 - Home Help Caregivers)
- 6Individuals with mandatory excludable convictions (the four categories in MSA Bulletin 19-03 / 42 USC 1320a-7[a]) are prohibited from serving as a Home Help caregiver; permissive exclusions bar enrollment unless the client signs the MSA-119 Personal Choice acknowledgement.(Adult Services Manual (ASM) 135 - Home Help Caregivers)
- 7Caregivers must revalidate their CHAMPS enrollment at least once every three to five years based on risk category, or enrollment is terminated.(Adult Services Manual (ASM) 135 - Home Help Caregivers)
- 8The individual caregiver must submit a monthly electronic service verification (ESV), or a paper service verification (PSV) if unable to submit electronically, to confirm Home Help services were provided.(Adult Services Manual (ASM) 135 - Home Help Caregivers)
