Rule 5160-45-04 | ODM-administered waiver program: provider enrollment process

What this rule requires your agency to do

  • 1At revalidation or when applying for a specialty, the provider must satisfy the criminal record check requirements set forth in Chapter 5160-45 of the Administrative Code.(Rule 5160-45-04 | ODM-administered waiver program: provider enrollment process)
  • 2Provider applicants must successfully complete the enrollment process and receive ODM approval before furnishing services to a waiver individual; services furnished before approval and before being added to the person-centered services plan are not reimbursable.(Rule 5160-45-04 | ODM-administered waiver program: provider enrollment process)
  • 3The provider applicant must submit a provider application to ODM in accordance with rule 5160-1-17 of the Administrative Code and meet all applicable waiver provider regulations.(Rule 5160-45-04 | ODM-administered waiver program: provider enrollment process)
