N.J. Admin. Code § 10:60-2.3 - Plan of care

What this rule requires your agency to do

  • 1The plan of care must include the specified elements, including diagnoses, case history summary, period covered, number/nature of visits, physician orders and changes, medications/treatments, equipment/supplies, long- and short-term goals, and discharge planning.(N.J.   • 2The plan of care must be re-evaluated by the nursing staff at least every 60 days and revised as necessary to the beneficiary's condition.(N.J. Admin. Code § 10:60-2.3 - Plan of care)
  • 3An interdisciplinary plan of care must be developed by agency personnel in cooperation with, and approved by, the attending physician/practitioner, covering medical, nursing, therapies, nutrition, home health aide services, and social care.(N.J. Admin. Code § 10:60-2.3 - Plan of care)
  • 4The plan of care must include an assessment of the beneficiary's acceptance of illness and receptivity to services, a determination of psycho-social needs relative to community resources, and a description of social services when provided by a social worker.(N.J.
